Planned Enrollment Form

    Due to federal loan proration changes, all EMPA students with federal aid eligibility must complete the 2026–2027 Planned Enrollment Form before we can package you with federal financial aid. This form asks you to indicate your current or planned enrollment for Summer 2026, Fall 2026, and Spring 2027. We understand that your Fall 2026 and Spring 2027 enrollment may not be finalized at this time; however, we need your best estimate so that we can package your aid as accurately as possible based on current federal loan eligibility and proration requirements. Please note that federal loan eligibility may be adjusted if you are enrolled below full time, which is a minimum of 12 credits per term. If your actual enrollment changes after submitting the form, you must notify the SIPA Office of Financial Aid and submit a revised Planned Enrollment form. Increasing or reducing your credits may result in a change to your federal aid eligibility.

    IMPORTANT NOTE: If your enrollment changes at any point after this form is completed, you must contact the SIPA Office of Financial Aid immediately and complete a revised form. Increasing or reducing the number of actual credits per term will result in a change in your federal aid eligibility.
